*Payments are non-refundable or transferable. In the event of cancellation of surgery or reschedule, a credit will be applied for up to 12 months for service. It is the client’s responsibility to notify of any changes at least 1 week before reservation in order to keep credit in good standing. No call no show will result in automatic cancellation. Only one date change can be made. All balances must be paid in full prior to being serviced. In the event that a deposit is lost, a new deposit will need to be placed. By placing a deposit, you agree to the terms and conditions stated above.
